Job Description:
A company is looking for a Continuing Care Specialist to support
individuals transitioning through different levels of care, guiding
them toward long-term stability and well-being.
Key Responsibilities
Provide essential resources and advocacy to clients navigating
their care journey
Communicate with external referral sources and coordinate seamless
transitions between programs
Foster relationships with adolescents and their families to promote
healing and joy
Required Qualifications
At least one year of experience in the behavioral health industry
in a relevant role
Ability to develop trusting relationships with families and care
providers
Technology savvy, with experience in video technology and
telehealth platforms
Preferred Bachelor's degree in mental health or a related field
Ability to pass a state background check
